What is FRCEM?
The Fellowship of the Royal College of Emergency Medicine (FRCEM) is the highest postgraduate qualification
awarded by the Royal College of Emergency Medicine (RCEM) in the UK. It is the definitive credential for doctors
pursuing consultant-level practice in Emergency Medicine in the United Kingdom and Ireland.
FRCEM builds on the Membership qualification (MRCEM) and assesses readiness for independent consultant-level
decision-making — encompassing advanced clinical management, leadership, governance, research literacy,
and complex resuscitation. The Fellowship consists of two independent components that can be taken in either order:

FRCEM SBA
Single Best Answer theory exam — 180 questions across two 2-hour papers, mapped to the full RCEM 2021 Emergency Medicine Curriculum. Taken at Surpass test centres worldwide.

FRCEM OSCE
Objective Structured Clinical Examination — 16 eight-minute stations assessing consultant-level clinical and communication skills. Delivered in-person in London only.
🗂 History note: Prior to August 2021, the Fellowship also included the FRCEM Critical Appraisal and FRCEM QIP components. Both have since been withdrawn as standalone examinations. Their competencies are now embedded within the SBA and OSCE.
Eligibility Requirements
Candidates must meet all requirements at the point of application, not at the date of the exam. Both FRCEM components share the same core criteria.

OrderCandidates may sit the FRCEM SBA and FRCEM OSCE in any order — there is no mandatory sequence between the two components.
⚠️ Currency rule: After completing MRCEM, candidates have 7 years to take and pass their first FRCEM component. Once they have passed one FRCEM component, they have a further 7 years to complete the remaining component.

Computer-based, at Surpass test centres worldwide. RCEM's calendar lists one more sitting in 2026 (7 October), then two a year from 2027.

OSCE Oversubscription Priority
When demand exceeds places, seats are allocated in this order:
- Candidates in approved ST6 UK training posts
- Candidates in Year 7 of an approved Irish training programme
- Candidates with an NHS employment contract
- Candidates not allocated a place at the most recent sitting
- All other applicants — first come, first served
⚠️ Do not miss the OSCE window. The 7-day window closes automatically at 16:00 UK time. Missing it means waiting for the next sitting — potentially 6 months away. Set a calendar reminder for 10:00 UK time on the opening date as soon as dates are published on the RCEM Exam Calendar.
Exam Fees
Fees depend on your RCEM membership status — members pay significantly less. A tax surcharge may be added
based on your country of residence (e.g. India has an 18% OIDAR government tax on all online RCEM services).

FRCEM SBA — Worldwide via Surpass
The FRCEM SBA is computer-based and taken at Surpass test centres worldwide — Surpass works with a global test-centre network to deliver RCEM's exams. Check which centres near you are offered when you book.
FRCEM OSCE — London Only

The FRCEM OSCE is only delivered in London. All candidates — including international doctors — must travel to the UK. This is a key logistical and financial consideration for IMGs.

Pass Rates
In RCEM's 2024 data, 51.1% of FRCEM SBA candidates and 80.6% of FRCEM OSCE candidates passed.

The pass mark is determined by the modified Angoff method and varies each sitting. One Standard Error of Measurement is added to the cut score.
FRCEM OSCE
RCEM's FRCEM OSCE pass rate was 80.6% in 2024 (562 of 697 candidates) and 83.9% in 2023. RCEM reports pass rates by candidate group only across all its exams combined, not for the FRCEM OSCE alone.
📊 Key insight: The FRCEM OSCE assesses consultant-level practice — not simply clinical knowledge. Examiners expect leadership, governance awareness, and decisive resuscitation capability. Many candidates who pass MRCEM OSCE underestimate the step up required for FRCEM.
Attempts Allowed & Currency Rules

4 attempts per component. FRCEM SBA and FRCEM OSCE are each limited to 4. Attempts taken after August 2016 count towards this limit.

After completing MRCEM: 7 years to take and pass your first FRCEM component.

Additional Attempts
After four failed attempts you may apply for one additional attempt, which RCEM's Chief Examiner considers case by case. Candidates with a new or previously undeclared condition needing reasonable adjustments, who failed earlier attempts without them, can also apply for additional attempts.

If you are running short of time, ask RCEM (exams@rcem.ac.uk) about an extension — they are considered case by case, for example after parental or long-term sick leave.
Validity & Recognition
What FRCEM lets you do depends on each country's regulator. Where we have checked the rules at source:

United Kingdom
The standard fellowship for EM consultants. Required for a CCT in emergency medicine; not mandatory for the GMC's portfolio pathway (formerly CESR), though RCEM strongly encourages it.
🇦🇪
United Arab Emirates
On its own, an exam-based Royal College fellowship is Tier 3 in the UAE's unified PQR: the Specialist title after three years' experience, not Consultant. A UK CCT is Tier 1, and GMC specialist registration together with the fellowship is Tier 2.
🇲🇾
Malaysia
FRCEM is on the Malaysian Medical Council's list of registrable emergency medicine qualifications. Specialist registration also needs completed specialised training that the MMC approves or accepts, and supervised work experience.
🇦🇺
Australia & New Zealand
ACEM assesses specialist international medical graduates who have completed specialist training and are recognised as emergency medicine specialists in their country of training.
Elsewhere — including Ireland, Saudi Arabia, Qatar and India — ask the regulator how it treats FRCEM before you plan around it.
Pros & Cons of FRCEM

- Reduced exam burden — Critical Appraisal and QIP components withdrawn since 2021

- OSCE in London only — international candidates must travel, with significant cost and visa requirements
- Only 4 attempts per component — fewer than MRCEM (6 attempts)
- 7-year currency windows create time pressure for non-trainees
- SBA pass rate of 51.1% in 2024 (47.0% in 2023, RCEM)
- Consultant-level expectation — the step up from MRCEM is significant and frequently underestimated
- Limited OSCE places with oversubscription risk for non-trainees
- Cost — exam fees plus UK travel and accommodation make it expensive for international candidates

How to Prepare for FRCEM
FRCEM requires a step-change from MRCEM preparation. The exam assesses consultant-level judgement, governance, and clinical autonomy — not just core EM knowledge.

Map everything to the RCEM 2021 SLOs
Download the RCEM EM 2021 Curriculum before opening any textbook. Every SBA question and OSCE station maps to a specific SLO. Understanding the blueprint lets you allocate revision time strategically — particularly to the high-yield SLO3 (resuscitation, ~40 questions) and SLO5 (paediatrics, ~30 questions).

Do not neglect SLO10, SLO11, and SLO12
Research & statistics, quality improvement, and education together account for approximately 20 questions. Learn basic statistics (NNT, likelihood ratios, sensitivity/specificity), QI methodology (PDSA cycles), and teaching frameworks (Pendleton's rules, SNAPPS).

Know UK-specific guidelines — not just global ones
The FRCEM is mapped to NICE, RCEM, BTS, JBDS, and RCOG guidelines. Many IMGs know the clinical management but apply non-UK protocols and lose marks. Revise NICE guidance for ACS, sepsis (NICE NG253 for 16 or over, NG254 for under 16s, NG255 in pregnancy), stroke, PE (PERC/Wells/CTPA pathway), DKA, and paediatric fever (NICE NG143).

Prepare for the OSCE as a consultant — not a registrar
The FRCEM OSCE is set at consultant level. You will be expected to lead a team, make governance decisions, delegate appropriately, deal with complex ethical scenarios, and demonstrate patient safety leadership. MRCEM OSCE preparation alone is insufficient.

Master resuscitation — it is a mandatory pass requirement
You must pass at least one resuscitation station to pass overall. Practise ALS, paediatric resus (APLS), major haemorrhage protocol, and RSI until they are automatic. These stations reward preparation and penalise hesitation heavily.

Practise SBA timing rigorously
90 questions in 2 hours = ~80 seconds per question. Many candidates run out of time. Practice timed mock papers from the start — develop a strategy for flagging and returning to difficult questions efficiently.

Book your OSCE early and plan your travel to London
The FRCEM OSCE is held only in London, and when a sitting is oversubscribed RCEM allocates places by its oversubscription criteria. International candidates should plan visas, flights and accommodation well in advance, and apply within the one-week window, which opens at 10am UK time.

Become an RCEM member before applying
Membership saves significantly on all FRCEM fees. Apply at rcem.ac.uk at least 24 hours before the application window opens — payment must be processed and reflected in your account before you can access member rates.

Respect the 4-attempt and 7-year limits — plan strategically
With only 4 attempts per component and 7-year currency windows, strategic planning is essential. Do not defer attempts indefinitely. Each failed attempt consumes one of your four — running out of attempts within the currency window is a career-limiting outcome.

Review your RCEM feedback letters after every attempt
RCEM provides detailed feedback letters after all FRCEM exams. Download your feedback from your RCEM account and discuss it with a supervisor or mentor. Domain-specific weakness data is invaluable for focused remediation before your next attempt.
